Job Summary
Provide whole-person integrated care management services to enrolled members, including comprehensive assessments and individualized care plans, care team development and ongoing care coordination, transitional care management, health promotion and medication monitoring, addressing unmet health-related resource needs, promoting a system-of-care approach, and 24/7 on-call crisis support (rotational). This full-time, hybrid role in Charlotte, NC requires a Bachelor's degree in a Human Services field or RN licensure, and a minimum of 2 years of direct experience with individuals with mental health, substance use, or behavioral health conditions, plus a valid driver’s license. Required Qualifications
