Job Summary
Systems Navigator (SN) for the Exceptional Family Member Program (EFMP) at MacDill AFB, FL. The role involves daily coordination to connect Families with special needs to required care, including intake interviews, needs assessment, referrals, follow-up, advocacy, and collaboration with military and civilian providers. duties include home visits to Soldiers and Family members, coordinating with government and civilian providers for respite resources, maintaining a catalog of networks, leading monthly support groups or workshops, presenting EFMP services briefs, providing information and referral services, partnering with Families to boost resilience and self-advocacy, managing Family/ Soldier records per Army/DoD regulations, ensuring smooth transitions, collecting data for planning and quarterly/monthly reports, and implementing policies to connect Families to support services. Required skills include knowledge of human service principles, familiarity with military and federal regulations around medical, educational, and Family support services, ability to facilitate non-clinical case management, strong communication, Life Coach certification, travel ability, proficiency with Microsoft Office, US work authorization, and background check clearance.
Required Qualifications
- Bachelor's degree in social work, psychology; marriage, family, and child counseling; counseling or behavioral science; education; community health or public health
- Two years of experience in family related case management and/or social work
- Level 1 Life Coach certified through Certified Life Coach Institute (Provided by Patriot)
- Must be able to travel
- Experience with Microsoft Suite programs
- Must be legally authorized to work in the United States
- Must be able to pass a background check
