Job Summary
Systems Navigation Support Worker within the My Health Team network provides linkage for clients to primary care services and community resources, supporting navigation across health and social systems. Responsibilities include skill-building, creating and maintaining support networks, identifying and connecting to community-based resources, outreach to homebound clients, and working across clinical and community settings to integrate health promotion, disease prevention, and self-management support. The role requires collaboration with primary care providers, community partners, and multidisciplinary teams, with responsibilities spanning information provision, crisis intervention concepts, and maintaining formal/informal support linkages to improve client access and continuity of care.
Required Qualifications
- University or College education in human services, other related disciplines, or high school diploma and related training/experience
- Requires a valid driver’s license and a vehicle
- Two years of related experience with people with mental health issues, or other human service
- Experience/education with de-escalation techniques
- Experience in life skill teaching and support to individuals who struggle with mental health issues or other disabilities or conditions
- Experience in developing, planning and coordinating services
