Job Summary
As a Sweet Dreams CNA/HHA, provide hospice-focused one-on-one care under the supervision of a Registered Nurse. Deliver bedside and personal care including hygiene, grooming, changing bed linens, aroma therapy, and comfort measures; assist with bathing, dressing, ambulation, and activities per the plan of care. Record vital signs (temperature, pulse, respiration, blood pressure) and report any significant changes to the Case Manager. Communicate effectively with patients, families, and the care team while upholding confidentiality and ethical care standards. Maintain a clean, orderly living area and participate in quality assessment and performance-improvement activities. Demonstrate empathy, tact, and calm demeanor when visiting patients’ homes and be flexible to ensure care is provided as outlined in the plan of care. Additional duties as assigned, with responsibilities supporting comfort, dignity, and family support in a hospice setting.
Required Qualifications
- Certification of Nursing Assistant or Home Health Aide required per state regulations
- Licensed driver with automobile that is insured in accordance with organization/state requirements and is in good working order
- Current CPR certification
- Willingness to submit to a criminal background check prior to hire and annually thereafter
- Minimum 75 hours of classroom and supervised practical training
