Sweet Dreams CNA/HHA role at Bristol Hospice in WI (Beaver Dam & Columbus area). You will join the Interdisciplinary Group Team, work under an RN supervisor, and perform bedside hospice care including personal hygiene, bed linens, oral care, back rubs, aroma therapy, and other comfort-focused tasks. Responsibilities include recording vitals (temperature, pulse, respiration, blood pressure), assessing patient condition and reporting changes to the Case Manager, assisting with ambulation and activities as outlined in the plan of care, and maintaining a clean, respectful patient environment. The schedule is Monday–Friday, 12pm–8pm, with duties aligned to the Hospice program’s care standards and quality-improvement activities. Requirements include state-regulation CNA/HHA certification, CPR certification, a valid driver’s license with insured vehicle, ability to pass background checks, and prior experience in hospice or community health as preferred. The role emphasizes compassionate, autonomous care, effective communication with patients and families, and adherence to confidentiality and organizational care procedures.
