Job Summary
Conduct dual discipline in-person social work assessments to evaluate MI Choice Program eligibility, identifying resources and coordinating participants to needed services and long-term care strategies. Facilitate person-centered planning by determining participant goals, preferences, and needs, then develop and revise Person-Centered Service Plans while monitoring and evaluating ongoing support. Maintain accurate records in electronic medical systems, complete case transfers following audits, and establish cooperative relationships with physicians, hospitals, and human service providers. Participate in required team meetings, trainings, and quality activities to ensure adherence to departmental standards and program metrics.
Required Qualifications
- Professional Social Work License (State of MI)
- Bachelor's Degree in Social Work & LLBSW OR Masters Degree in Social Work & LLMSW
- Valid drivers license
- Competence in Microsoft Office
- 40wpm typing
- Ability to work in a community-based setting, independently with little direct supervision of daily duties

- Experience managing a caseload of persons with diverse needs
- Knowledge of available community services
- Experience working with older adults
