Job Summary
Lead care coordination by facilitating discharge planning, eliminating barriers, and prioritizing moderate to high-risk patients with complex psychosocial needs. Act as a patient navigator and advocate, providing referrals and ensuring clear communication with caregivers regarding transitions of care. Collaborate with multidisciplinary teams to develop post-acute care plans and back-up strategies. Participate in data collection, clinical audits, and programmatic evaluations focused on length of stay and readmissions. Requires a Master's in Social Work, current Ohio license, and experience with diverse populations. This role supports Kettering Health's mission to restore health across southwest Ohio's integrated system.
Required Qualifications
- Masters of Social Work or related field
- Current Ohio Social Work license
- Clinical experience in health care setting
- Awareness of developmental needs over the age span of infant to elder
- Ability to work well in multidisciplinary and independent settings and communicate well (verbal and written)
- Ability to coordinate work effectively with other hospital disciplines
- Ability to recognize and understand the dynamics of individuals from diverse populations
- Ability to solve problems through the analysis of information
