Job Summary
Coordinate services for moderate and high-risk patients with complex health problems or psychosocial issues, prioritizing discharge barriers and post-acute care planning. Act as a patient advocate and navigator by providing information, referrals, and advocacy to address psychosocial needs while promoting clear communication with the care coordination team. Collaborate on data collection, health outcomes reporting, and clinical audits to support organizational goals for length of stay and readmissions. This role requires a Master's in Social Work and an active Ohio license, with an 8-hour first shift schedule at Kettering Health Behavioral Medical Center in Moraine.
Required Qualifications
- Masters of Social Work or related field
- Current Ohio Social Work license
- Clinical experience in health care setting
- Awareness of developmental needs over the age span of infant to elder
- Ability to work well in multidisciplinary and independent settings
- Ability to communicate well (verbal and written)
- Ability to coordinate work effectively with other hospital disciplines
- Ability to recognize and understand the dynamics of individuals from diverse populations
- Ability to solve problems through the analysis of information
