Job Summary
Coordinate the full continuum of care for patients undergoing surgical procedures, including total joint replacements, ensuring a seamless experience from initial consultation through post-operative recovery. Serve as the central point of contact for patients, providers, and interdisciplinary teams to facilitate timely access to care, promote surgical readiness, and support optimal clinical outcomes while aligning with Marshall's standards for quality, safety, and patient experience. Maintain current California LVN licensure and BLS certification, leveraging at least three years of orthopedic or surgical services experience. Work primarily during days shifts in El Dorado County, with proficiency in EPIC systems preferred.
Required Qualifications
- Current California LVN License
- Current BLS certification
- Minimum three (3) years in orthopedic, surgical services, or care coordination
Desired Qualifications
- Prior experience in care coordination, navigation or population health
- Familiarity with pre-operative clearances workflows and post-acute coordination
- Proficiency in EPIC
