Job Summary
Complete assessments to identify participant needs and desires for service delivery. Develop and modify Person-Centered Service Plans annually or upon significant changes. Coordinate services by brokering housing, medical, social, and vocational supports while ensuring Behavioral Support and Crisis Plans align with Individual Support Plans. Actively collaborate with care coordinators and entities to ensure seamless delivery of physical and behavioral care. Report incidents of abuse, neglect, or exploitation immediately to supervisors. Research alternative solutions for participant needs and maintain confidentiality regarding sensitive health information. Confers regularly with supervisors to address participant abilities and limitations. Travel 50% of the time to participants as needed.
Required Qualifications
- BS/BA degree in related course work
- Minimum of 3 years related experience providing case management for individuals with Intellectual Disabilities, Autism Spectrum Disorders, or Physical Disabilities
- Experience with Microsoft Office, state-maintained databases
- Must maintain proficiency in company sponsored training and certifications
- Maintain CPR/First Aid certification and updated state related Clearances
- Successfully complete and maintain training courses as required or amended by program regulations
- Valid Driver's License including personal vehicle insurance coverage for liability and current vehicle registration, copies must be maintained on file with Care Lync
- 50% travel to participants as needed
