Job Summary
Coordinate and manage Service Coordination/Case Management for adults with Intellectual/Developmental and Physical Disabilities. Develop and implement Person-Centered Service Plans (ISPs), broker and monitor services, ensure alignment of plans with participant goals, and evaluate clinical outcomes. Maintain thorough documentation, ensure confidentiality, coordinate with care teams and families, and participate in required trainings. Responsibilities include assisting with housing, medical, social, vocational services, and supporting overall independence in home and community; travel up to 50% as needed; requires a BS/BA and proficiency with Microsoft Office and state databases; bilingual English-Spanish encouraged; CPR/First Aid certification and state clearances maintained.
Required Qualifications
- BS/BA degree in related course work
- Experience with Microsoft Office, state-maintained databases
- Ability to add, subtract, multiply and divide in common units or measure, using whole numbers, common fractions, and decimals
- Must maintain proficiency in company sponsored training and certifications
- Valid Driver's License including personal vehicle insurance coverage for liability and current vehicle registration
