Job Summary

Develop Individual Plans of Service through person-centered planning and provide ongoing linking, coordinating, and monitoring of services for clients with developmental disabilities. Conduct in-person meetings within the Kent County community while maintaining remote work responsibilities. Manage case coordination to ensure continuous support for individuals with intellectual disabilities, substance use disorders, or mental illness. Maintain a valid driver's license for fieldwork and demonstrate strong active listening and time management skills. Work Monday through Friday from 8:00am to 5:00pm in a hybrid role.

Required Qualifications

  • Bachelor's degree in human services field
  • one year of experience treating or working with a person who has intellectual disability
  • specialized training received through fieldwork and/or internships
  • Possession of a valid driver's license
  • access to reliable transportation for job related use
  • Strong organizational skills
  • experience in person-centered planning
  • Knowledge of Culture of Gentleness
  • Demonstrated understanding of persons with developmental disabilities
  • Demonstrated communications skills that effectively convey information
  • Demonstrated active listening
  • Demonstrated time management skills
  • Lived experiences with mental illness/developmental disabilities/substance use disorders

Desired Qualifications

  • Licensed BSW
  • Knowledge of trauma-informed theories, principles, and practices (includes multi-faceted understanding of concepts such as community trauma, intergenerational and historical trauma, parallel processes, and universal precautions)
