Job Summary
Supportive Services Specialist for Sea Mar’s Affordable Housing program in the Beacon Hill area of Seattle. Conduct initial screening and comprehensive psychosocial assessment; develop and implement care plans; check in with residents about their care plans via discussion or counseling; consult with the interdisciplinary supportive services team on client needs; maintain client-centered, culturally competent care; provide crisis intervention, address mental health concerns, pregnancy and birth options, grief, coping skills, problem-solving, abuse/neglect, and family violence; complete thorough psychosocial assessments covering personal history, family functioning, support systems, housing stability, and health needs; make referrals to appropriate agencies or services; assist clients with accessing services (forms, documentation); document encounters and update assessments; coordinate with co-workers and external agencies; ensure compliance with state and agency policies; document data for the supportive services program; participate in outreach efforts; maintain professional standards; and perform other duties as assigned. Will require ability to work with an interdisciplinary team, use EHR systems, and meet credentialing and vaccination requirements.
Required Qualifications
- Background check required (Washington State Patrol background check)
- Must have access to a car and valid driver’s license
- Must pass pre-employment Motor Vehicle Record (MVR) check
- Proof of minimum liability auto insurance coverage
- Must be fully vaccinated against COVID-19 or have a medical/religious exemption
- TB test required at time of hire and annually
- Basic Life Support (BLS) CPR certification within 90 days of hire and maintained
- Bachelor’s level degree in Counseling, Social Work, Psychology or related field preferred
- Must pass a background check and meet safety requirements for working with vulnerable populations
- Prefer Agency Affiliated Counselor in WA and Certified Peer Support in WA
- Typing proficiency 35 wpm (implied as required)
- Experience with electronic health records and Microsoft Office tools
