Job Summary
Coordinate transportation resources and educational services to ensure access to healthcare; assess transportation needs, determine best mode of eligible transportation, and develop plans that address educational needs. Manage and document transportation resources (bus passes, gas cards, cab rides) and educational services, maintain records in electronic health records and AIRS, and collaborate with internal and external providers to improve health outcomes. Travel to rural areas if needed and participate in case conferences and reassessment processes to build client independence and self-management skills.
Required Qualifications
- Bachelor's degree in health & human services, Social Work, or a closely related field with 1 year of related experience
- Associate degree in Health & Human Services, Social Work, or a closely related field with 2 years of related experience
- High school diploma with 3 years of related experience
