Job Summary
Provide housing-focused case management to residents of Permanent Supportive Housing communities, developing individualized service plans and maintaining regular contact. Respond to referrals from property management, community partners, and residents to assist with lease compliance, rent arrears, and behavioral health concerns. Connect residents to healthcare, substance use treatment, employment, and benefits while coordinating services with healthcare providers and agencies. Deliver crisis intervention and support using Housing First, trauma-informed, and strengths-based practices. Maintain accurate HMIS records, documentation, and program reporting. Participate in case conferencing, multidisciplinary meetings, and quality improvement activities. Maintain a full caseload and complete required service contacts within established timelines. Work both in an office environment and on-site at residential units, occasionally driving to resident locations.
Required Qualifications
- Associate degree in Social Work, Human Services, Psychology, Sociology, or a related field
- Valid driver's license
- Reliable transportation
- Knowledge of Housing First
- Knowledge of trauma-informed care
- Knowledge of motivational interviewing
- Knowledge of harm reduction
- Strong communication skills

Desired Qualifications
