Job Summary
Provide individually tailored services and activities within the consumer's home and community to assist in acquiring, retaining, and improving self-help, socialization, and adaptive skills. Train individuals in independent living skills, home management, cooking, cleaning, and supported employment while ensuring services align with person-centered plan objectives. Report incidents within 24 hours, complete activity logs, and remind consumers to take or refill medication as necessary. Maintain compliance with personnel documents and annual training requirements. Must hold a valid driver's license, possess current auto insurance, and be physically able to drive to various locations in Arkansas.
Required Qualifications
- High School diploma OR GED
- 1 year relevant supervised experience (public health, human services or other community service agency)
- The ability to understand written activity plans, execute instructions, and document services delivered
- Have the ability to communicate effectively with consumers
- Have the ability to access emergency service systems
- Have the ability to access transportation services required as appropriate
- Have satisfactorily passed a criminal background check, and adult and child maltreatment registry checks
- Criminal background and adult maltreatment checks must be repeated every five (5) years, and child maltreatment checks must be repeated every two (2) years
- Have completed a drug screen prior to employment in accordance with the organizations policies and procedures
- Must possess current valid driver's license
- Have current auto liability insurance coverage and registration
- Be physically able to drive to various locations in Arkansas
- Must occasionally lift and/or move up to 50 pounds
- Specific vision abilities required by this job include close vision; distance vision; color vision; peripheral vision; depth perception; and ability to adjust focus
