Job Summary
Assist tenants in securing safe, affordable housing and establish utilities, furniture, and household items. Initiate monthly check-ins to review living environments and monitor mental health status while documenting all visits and contacts. Provide teaching, monitoring, and recovery-oriented supports to help tenants maintain independence, settle housing disputes, and ensure all entitlements are secured. Coordinate moving in and out of units, provide transportation when necessary, and ensure tenants maintain clinical services, vocational education, and HUD/Section 8 applications. Create individualized housing support plans with tenants and their service providers. Maintain adequate records, provider reports, and statistics as requested by the supervisor.
Required Qualifications
- Bachelor's degree
- 4 years of mental health experience
- clean NYS driving record
- use of one's personal vehicle
- at least Eighteen (18) years of age
Desired Qualifications
- One year experience working in human services field
