Job Summary
The Supportive Housing Program Support Specialist position focuses on providing comprehensive support services to homeless and vulnerable clients with significant medical needs. Responsibilities include case planning, advocating for clients, facilitating referrals to service providers, maintaining case files, and participating in case conferencing. Candidates should have experience in human services, especially in supportive case management for at-risk populations, and possess excellent communication skills. A valid driver’s license and car insurance are required.
Required Qualifications
- High School Diploma, GED or equivalent experience
- At least two years’ experience in Human Services field providing supportive case management services
- Knowledge of working with vulnerable populations
- Valid Driver’s License
- Valid car insurance
Desired Qualifications
- Bilingual or ASL ability
