Job Summary

Conduct outreach-based support for individuals in Permanent Supportive Housing grants to stabilize housing and connect to community resources. Collaborate with participants to assess needs across medical, mental health, substance use, and basic services, while coordinating with landlords to mitigate tenant issues. Maintain accurate participant files in HMIS, document service delivery, and prepare monthly case management reports. Adhere to allocated budgetary guidelines and attend required staff and case management meetings. This temporary, full-time role at the Macadam Office in Portland emphasizes harm reduction and individualized support for vulnerable populations.

Required Qualifications

Bachelor's Degree in a related field
2 years of experience in social services
Demonstrated ability to interact with other agencies in developing and providing services to participants
Ability to advocate on behalf of participants for services with other agencies
Demonstrated knowledge of chemical dependency and mental health issues and appropriate treatment resources
Ability to identify and work effectively with property and building managers
Ability to develop positive long-term relationships with property and building managers
Ability to provide mediation between agencies, participants, and property and building managers
Up-to-date familiarity of community resources and services available to assist the homeless
Demonstrated knowledge of domestic violence and sexual assault issues and resources
Demonstrated knowledge of services available to assist the homeless
Demonstrated knowledge of services available to assist veterans
Demonstrated ability to work as part of a team in the delivery of services to participants
Computer literacy
Demonstrated ability to use word processing, database, spreadsheet, e-mail, and Internet programs
Demonstrated ability to maintain accurate and comprehensive participant files
Demonstrated ability to prepare accurate and comprehensive statistical and narrative reports
Valid driver's license
