Job Summary
Provide case management and life skills development for homeless individuals and families. Conduct assessments and interviews to determine eligibility, develop and update action plans and documentation, connect clients to medical, mental health, and supportive services, coordinate referrals, guide clients through SSI/SSDI/Medicaid/Medicare/SNAP processes, conduct occasional home visits, coordinate with property management for wellness checks, track and report outcomes, and travel within the area to assist clients. Role requires in-office work with some nights and weekends, driving as needed, and ability to lift up to 50 lbs. Qualifications include a Bachelor's Degree in social services or related field with related experience, or a High School Diploma/GED with two years of related experience; bilingual preferred. Must have valid driver's license and use of vehicle. Position is non-remote with in-office duties and community outreach.
Required Qualifications
- Bachelor's Degree in social services or related field and a minimum of one year experience in the coordination of supportive housing or in the coordination of independent living services in a social service setting
