Job Summary
Provide intensive case management and care coordination for residents of permanent supportive housing, linking to mental and primary health care, substance-use treatment, financial management, and employment services. Develop creative strategies to engage residents, de-escalate crises, conduct outreach, and connect residents to resources. Collaborate with Property Management to align policies and safety efforts; document delivered services for grant reporting and program administration. Position requires related experience in homelessness services, mental health/crisis intervention, and strong communication and organizational skills. Master of Social Work degree (or related field) recommended; bilingual abilities helpful.
Required Qualifications
- Master of Social Work Degree (or related field) recommended
- At least one year of experience in providing intensive case management services to adults experiencing homelessness, exiting homelessness, or at risk of homelessness
- At least one year of experience in mental health/substance use and crisis intervention
- Ability to work independently, problem solve, think critically and independently and effectively communicate with people from diverse backgrounds
- Strong interpersonal skills, good organizational methods and multitasking
- Spanish or ASL proficiency helpful (preferred)
