Job Summary
Supported Living Specialist responsible for hands-on training and support to individuals to develop daily living skills, nutrition, mobility, and other independence-building activities. Responsibilities include developing and implementing a Supported Living Plan, training ADL skills, maintaining documentation, providing direct care, coordinating with government/local agencies, assisting with housing placement, creating Implementation and Goal plans, maintaining PBHC/Medicaid Waiver records, documenting services, providing 24/7 emergency availability, assisting with personal care, and fulfilling scheduled duties. Requires excellent communication, interpersonal, time-management skills, flexibility, and ability to work with interdisciplinary teams; proficient with Microsoft Office and office equipment; valid Florida Driver’s License and auto insurance; 18-hour pre-service certification; Bachelor’s degree in a human services field or equivalent health-services experience.
Required Qualifications
- Bachelor's Degree in Human Services related field or experience in the health services field shall substitute on a year for year basis for the required education along with a High School Diploma or GED.
- Eighteen (18) hour Supported Living pre-service certification required before serving program participants.
- Must possess and maintain a current Florida’s Driver’s License and state minimum requirements for auto insurance.
