Job Summary
Assist individuals with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and meal preparation. Administer medications following prescribed protocols and accurately document all medication-related activities. Collect and record data related to progress and service outcomes while supporting implementation of individualized Plans of Care (POCs). Transport individuals to appointments, community activities, and recreational outings. Promote independence, positive behaviors, and skill development by collaborating with families, case managers, and healthcare professionals to ensure safety, dignity, and well-being. Work 12-hour shifts in a 3-day rotation including weekends and holidays. Requires a valid Alaska Driver's License, high school diploma, and ability to pass a background check.
Required Qualifications
- Must be at least 21 years of age
- High School Diploma or equivalent
- Valid Alaska Driver's License with a good driving record
- Ability to pass a State of Alaska background check
- Reliable transportation
- Basic computer and documentation skills
- Ability to work a flexible schedule, including weekends
- Strong communication and interpersonal skills
- Ability to provide compassionate, person-centered care
- Patience, professionalism, and sound judgment
- Ability to learn and utilize EMR systems and other technology
- Commitment to supporting independence in a least restrictive environment
- Medium work with lifting up to 50 pounds occasionally
- Ability to assist with physical interventions as needed
- Ability to communicate clearly in emergency situations
- Annual PPD Test
- Hepatitis B vaccination or waiver
