Job Summary
Supported Living Facilitator to support people with disability in daily living, personal care, and autonomy-focused planning across Ashgrove, The Gap, Bracken Ridge and Mount Gravatt. Responsibilities include partnering with the person, their family and informal networks to attain life goals; providing support in everyday living situations; developing, implementing and reviewing individual plans; promoting personal autonomy and dignity; and upholding privacy and confidentiality. Desirable qualification in disability work is noted, along with required checks (NDIS Worker Screening and Blue Card), valid driver’s license, and CPR/First Aid. Sleepover shifts are available at a casual, not-for-profit setting with salary packaging benefits, and opportunities for flexible scheduling. Required Qualifications
