Job Summary
Coordinate and provide case management for the Supported Housing program in a New York nonprofit health and human services setting; drive an agency vehicle to visit apartments and assess residents' needs; liaison with community health, mental health, educational, vocational, and recreational programs; supervise and collect urine samples; provide linkage services with detoxification and rehabilitation centers; assist consumers in housing selection, subleases, and securing utilities; facilitate housing subsidies, benefits, and tenant-landlord communications; develop individual support plans and document all records; on-call 24/7 for emergencies; weekly supervision of staff; participate in trainings at various sites; CPR and First Aid training required; requires a Bachelor’s degree in Social Services (preferred) and a New York State driver’s license with access to a vehicle.
