Job Summary
Facilitate Targeted Case Management (TCM) services for up to 15 adult clients, providing 1-5 service hours per client monthly while maintaining a minimum of 50 billable services. Conduct job coaching, vocational training, and resume preparation to support clients in securing and maintaining employment, including market development and barrier removal. Deliver comprehensive support throughout the job search process, crisis intervention, and coordination with therapists, doctors, and community resources. Complete intake packets, care plans, and progress notes weekly, submit critical incident reports within 48 hours, and collaborate monthly with treatment teams. Participate in clinical supervision twice monthly and attend required training sessions. Maintain a caseload of 10 TCM clients and assist with housing, benefits, and somatic health management needs.
Required Qualifications
- Valid Maryland Driver's license
- Reliable and current auto insurance (State minimum for Maryland)
- CPR/ First Aid Certification
- Monday - Friday daytime (9am - 7pm) availability
- Shifts will be between 6-8 hours a day for 5 days a week
- Must have your own reliable transportation
- Must be able to meet clients face-to-face
Desired Qualifications
- Bachelor's Degree preferred
- 2 years' experience providing direct services to adults with emotional and behavioral challenges preferred, including vocational experience
