Job Summary
Provide personal care and support with daily living tasks for residents with autism, learning disabilities, or mental health needs. Encourage participation in activities and community life while assisting with assessment and implementation of individual care plans. Maintain accurate records, care plans, and daily notes to uphold safety, security, and confidentiality. Act as a key worker to help residents set goals, track progress, and promote choice and independence. Build positive relationships with service users, families, and colleagues while contributing to continuous improvement through shared ideas.
