Job Summary
Provide direct support, skill development, and community resource linkages to residents with psychiatric and/or substance use disabilities. Field-based work (70-80%) involves meeting residents on their service plans, assessing needs, and documenting progress notes to comply with regulatory standards. Build person-centered, collaborative approaches to enhance independence, advocate for residents, and connect them with housing, healthcare, employment, education, and other social services. Engage in advocacy, partnerships with community organizations, and trauma-informed support to promote self-sufficiency and successful community integration. Rotating weekend on-call shifts with added compensation.
Required Qualifications
- Bachelor's degree in a mental health-related discipline OR high school diploma and at least three years of relevant work or lived experience
- Ability to prepare clear, concise, timely, and accurate billable progress notes in accordance with clinical/regulatory requirements
- Valid NJ Driver’s License with acceptable driving record
