Job Summary
Support Worker in the Family Preservation Program providing direct support to families to promote child safety, stability, and well-being. Works with case managers, therapists, and community partners to ensure services align with case plans; responsibilities include direct/indirect support in home/school/community, coordinating resources (healthcare, childcare, housing, employment, benefits), crisis intervention and parenting guidance, assisting with SNAP/Medicaid/SSI and employment support, documenting activities for audits, addressing barriers to engagement, transporting clients and attending court proceedings when needed, and promoting teamwork and agency values while maintaining accurate documentation and communication with service providers.
