Job Summary
Provide personal care and support with daily living tasks, encourage participation in activities and community life, and help service users achieve their personal outcomes. Maintain accurate records, care plans, charts, and daily notes while building positive relationships with service users, families, and colleagues. Uphold safety, security, and confidentiality in line with policies and regulations, acting as a key worker to set goals and track progress. Promote choice, independence, and involvement at all times, contributing to continuous improvement by sharing ideas and suggestions. Work across four individual units within a registered care home serving individuals with autism, learning disabilities, and mental or physical health needs.
