Job Summary
Community-based support and advocacy role providing direct emotional support, crisis intervention, and resource navigation to individuals. Develop personalized support plans, collaborate with families and professionals, and facilitate group activities to promote socialization and skill development. Maintain confidential documentation and stay updated on resources, policies, and regulations. Requires a Bachelor's degree in social work, psychology, or related field (or equivalent experience); experience with diverse populations; strong communication and problem-solving skills; and a valid driver’s license with reliable transportation.
Required Qualifications
- Bachelor's degree in social work, psychology, or a related field (or equivalent experience)
- Proven experience working with diverse populations, including individuals with disabilities, mental health challenges, or other support needs
- Strong interpersonal and communication skills, with the ability to build rapport and establish trust
- Knowledge of community resources and services, and the ability to navigate and advocate within the system
- Ability to work independently and as part of a team, with excellent problem-solving and decision-making skills
- Proficient in using technology and software for documentation and communication purposes
- Valid driver's license and access to reliable transportation
