Job Summary
Conduct comprehensive services through on-site and off-site programs, including case management, psycho education, crisis intervention, advocacy, and community liaison. Complete all documentation such as shift logs, incident reports, and service notes. Monitor and educate clients on medication self-dosing while teaching community skills like ADLs in individual or group settings. Coordinate with Rehabilitation Counselors and the treatment team to schedule appointments and create an interdisciplinary approach. Travel locally between the office, client residences, and community settings. Maintain skills through 20 hours of annual training and adhere to all safety regulations. Work Monday through Friday from 7am to 3:30pm at Key Point Health Services in Glen Burnie, MD.
Required Qualifications
- High school diploma
- Support Staff should have either experience or education related to the population with whom they shall work
- knowledge of the DSM and relevant symptoms of the most common diagnosis within the RRP programs
- proficiency in advocacy skills and case management skills
- ability to express themselves well through writing, interpersonal and oral communication skills
- understanding and sensitivity to serving culturally diverse and/or special needs population while maintaining patient confidentially
