Job Summary
The Support Coordinator role involves assisting clients by assessing their needs for various support services and facilitating access to these services, including vocational, medical, and psychological support. Responsibilities include developing Individualized Service Plans (ISPs), monitoring services, conducting in-home visits, and ensuring that clients receive appropriate support. The position requires excellent organization and communication skills, computer proficiency, and the ability to work outside of regular hours and travel regularly. Candidates must possess a Bachelor’s Degree, one year of relevant experience, and a valid Driver’s License. The role may also evolve into an Assistant Supervisor position, requiring further experience in the I/DD field.
Required Qualifications
- Bachelor’s Degree in social or human services field
- One year of experience working with the adult I/DD population
- Ability to respond to calls and programmatic needs beyond regular work hours
- Must be at least (18) years of age
- Must hold a valid Driver’s License with no more than 5 points
- New Jersey State drug test screening is mandatory prior to employment
- Ability to travel very regularly (50 – 75%) throughout county areas
- Excellent organization and communication skills
- Above average computer skills, including ability to learn and work in NJ State mandated software programs
- Ability to work independently
Desired Qualifications
- At least one (1) year of experience working in case management
