Job Summary
Support Coordinator in the Developmental Disabilities Division (DDD) supports coordination of services for adults with DD/ID through documentation, scheduling, communication, and follow-up. Responsibilities include managing caseloads, preparing ISP-related documentation, data entry in DDD-approved databases, serving as primary contact for clients and families, coordinating services across healthcare, vocational, residential, and community settings, monitoring progress, distributing training materials, assisting in crisis situations, and ensuring audit-ready compliance by maintaining accurate service descriptions and hours. Ideal candidate demonstrates strong organizational, written and verbal communication skills, proficiency with Microsoft Office and databases, confidentiality aligned with HIPAA, and experience working with individuals with DD/ID. Bilingual ability (Spanish, ASL, or other languages) and knowledge of DDD services are preferred.
Required Qualifications
- Bachelor degree in Human Services, Social Work, Psychology, or a related field preferred
