Support Coordinator - Case Manager Social Services - York, PA

Coordinate and manage individualized care plans for adults with Intellectual/Developmental and Physical Disabilities under the Medicaid Waiver program. Develop and modify Person-Centered Service Plans (ISP), broker and coordinate services, monitor outcomes, and ensure services are provided in a professional, comprehensive, and cost-effective manner. Assist participants in identifying and communicating needs for housing, medical, social, vocational, and community supports; conduct assessments; ensure Behavioral and Crisis Plans are aligned with the ISP; report incidents of abuse, neglect, or exploitation; actively coordinate with other care coordinators and entities to ensure seamless care delivery; maintain confidentiality of sensitive information; participate in required trainings and certifications; travel to participant locations as needed; maintain bilingual communication when applicable.
