Job Summary
Coordinate services and support informed, person-centered decisions for children and adolescents with intellectual and developmental disabilities by developing individualized plans and connecting families to providers and community resources. Monitor service delivery and progress toward outcomes while advocating for needs across medical, educational, vocational, and recreational domains. Balance scheduled touchpoints with families and providers against time to update records, review plan milestones, and prepare for upcoming meetings. Navigate a fast-moving service landscape by shifting between problem-solving, coordinating next steps, and ensuring families have clear, practical information to make informed choices. Ensure supports remain aligned as needs change, helping maintain steady momentum toward successful transitions and discharge goals.
Required Qualifications
- Bachelor's degree in Human Services, such as Social Work, Psychology, or Counseling
- One (1) year of experience in support coordination services with the assigned population
- Must successfully pass a criminal history fingerprint background investigation
- Must successfully pass a DMV record check
- Must successfully pass a Child Registry search
- Must successfully pass a drug screening test
- Must successfully pass employment reference checks
