Job Summary

Devising personalised support plans to address challenges and enhance older people's quality of life. Coordinating access to ALONE's services and community partners while advocating for needs within health, social care, and welfare systems. Establishing referral relationships with statutory and non-statutory agencies to optimize service delivery and maintain accurate records in the CRM database. Managing technology distribution for safety and well-being, monitoring key performance indicators, and delivering timely reports on service developments. Full-time permanent role with a 35-hour week, core hours 9-5, requiring a clean driver's license and own car.
