Job Summary
Devising personalised support plans to address challenges and enhance older persons' quality of life. Provide access to ALONE's services while coordinating and enabling older people to access other community services. Advocate for needs within health, social care, and welfare services to streamline access. Establish and nurture referral relationships with statutory and non-statutory agencies, developing efficient pathways for Support Coordination services. Manage technology distribution and installation to improve well-being and safety. Maintain records on the CRM database to inform service delivery and strategies. Deliver thorough reports on service developments, challenges, and achievements. Monitor key performance indicators to achieve SMART targets. Requires a clean driver's license and own car. Full-time, 35 hours per week.
