Job Summary
Develop personalized care plans and connect individuals living with developmental disabilities to essential services and supports. Provide ongoing advocacy, counseling, crisis intervention, and daily living support to promote independence and community integration, including monitoring and coaching to address physical, financial, social, and personal needs. Requires a Bachelor's degree in Human Services and at least one year of case management or support coordination experience, plus a valid driver’s license. Criminal history fingerprint background checks and DMV/Child Registry checks are required during the selection process.
Required Qualifications
- Bachelor's degree in Human Services (Social Work, Psychology, or Counseling)
- One year of experience providing case management or support coordination services to adults living with a developmental disability
- Valid Driver's license
