Support Coordination/Psychosocial Recovery Coach Subcontractor (Limestone Coast)

Manage caseloads for individuals with psychosocial disabilities by navigating the NDIS, achieving personal goals, and enhancing independence. Deliver support across Level 2 Support Coordination, Psychosocial Recovery Coaching, and Specialist Support Coordination roles. Maintain a minimum of 15 billable hours per week while setting your own KPIs. You must hold an Australian Business Number, public liability and professional insurance, NDIS Workers Clearance, and DHS Working with Children Clearance. Provide transport using a fully insured vehicle and manage your caseload via smartphone and laptop. Work from home or access office sites as needed, with billing and invoicing handled by the organization.
