Job Summary
Conduct initial nursing assessments and supervisory visits for non-skilled home care patients, ensuring caregivers receive safe, compassionate care in their homes. Develop, review, and update individualized plans of care while supervising Home Health Aides and Personal Care Aides to maintain quality and regulatory compliance. Complete caregiver competency evaluations, educate staff on infection prevention, and monitor patient conditions to communicate changes with physicians and families. Maintain accurate clinical documentation and collaborate with administrative staff to ensure continuity of care. This PRN leadership role supports quality assurance initiatives and agency standards for Human Touch Home Health, which has served communities across Washington, DC, Maryland, Virginia, and Pennsylvania for over 27 years.
Required Qualifications
- Active Washington, DC Registered Nurse (RN) License
- Ability to work independently while providing guidance and support to caregivers
- Commitment to delivering exceptional patient-centered care

- Previous Home Health or Supervisory Experience
- Knowledge of District of Columbia home care regulations
