Supervisor of Care Management

Oversee and supervise care managers to implement and monitor standards of care while providing comprehensive care management services including outreach, assessment, and development of individualized care plans. Conduct meetings to communicate expectations and changes in policies, consult with primary care physicians to identify crisis intervention plans, and provide care coordination for medically necessary care and health promotion. Manage transitional care by following up on admissions and discharges from ER, hospital, or residential settings, facilitating discharge planning, and linking patients to community supports. Assist direct reports with key functions through coaching and resource provision, while maintaining confidentiality and adhering to agency policies and corporate compliance guidelines.
