Job Summary
Supervisor, Care Managers to lead and supervise a care management team delivering member-centered, regulated care for enrollees requiring medically necessary long-term services; responsibilities include monitoring performance, quality assurance, coaching staff, coordinating with providers and interdisciplinary teams, and driving improvements in care plans, transitions, and member outcomes, with emphasis on timely completion of assessments, documentation, and adherence to organizational, contractual, and state/federal regulations.
Required Qualifications
- Bachelor’s degree in a human service, social science, or health field
- Five (5) years’ experience in Care Management, including at least one year related to elderly and disabled populations
- Strong knowledge of long-term care related services, community resources, and relevant regulatory requirements
- Proficiency in documentation, reporting, and use of electronic health records or case management software
- Proficient in Microsoft Office applications
