Job Summary
Supervise care coordination activities to ensure appropriate services are delivered by the team and providers while maintaining continuity of care across services. Monitor and review care coordination documentation, including transition of care and service assessments, to ensure compliance with contractual requirements and regulatory guidelines. Assign caseloads to care coordination team members based on state and contractual requirements and member needs. Work with the care management team to address member needs, ensure standards of practice are compliant, and provide coaching to improve continuity of care. Evaluate team performance, provide feedback on goals, and assist with onboarding, hiring, and training. Lead regional teams, conduct in-field training, and travel up to 20% of the time for meetings and support.
Required Qualifications
- Bachelor's degree
- 4+ years of related experience

- RN
- 6+ years experience with populations served
- Driver's license
- Child and adult maltreatment check
- Drug screen
- Reside in AR or border city
- 20% travel
Desired Qualifications
- Experience working with Behavioral Health (BH) and/or Developmental Disabilities (DD) populations
