Supervisor – Assertive Community Treatment (ACT):

Supervise delivery of wrap-around case management, therapy, skill building, substance use, and crisis intervention services to adults with chronic mental illness and co-morbid conditions. Manage all aspects of team performance, including documentation, fidelity to the ACT model, accreditation, Medicaid standards, program outcomes, and clinical development. Guide collaboration between primary care, specialist care, and psychiatric providers while developing risk mitigation and crisis prevention strategies using a harm reduction approach. Contribute to program success through direct care to participants and cultivating a cohesive team. Work closely with community service providers to increase service access and strengthen relationships.
