Job Summary
Supervise Adult Care Assistant teams and manage ADHC recordkeeping while providing direct nursing care to participants under Center Manager oversight. Perform reassessments of Home Care services, coordinate durable medical equipment, PCA Services, and Nursing Facility Care, and ensure home environment safety. Requires good interpersonal skills and effective communication at all levels. Full-time role with competitive compensation.
