Job Summary
Supervisor, Acute Case Management supervises the department’s overall operations across multiple locations, collaborating with Nurse Managers/Directors, Risk and Ethics staff, Ancillary Services, Patient Access, Medical Staff leaders, physicians, Health Plans and contracted providers, and post-acute agencies to ensure compliant, high-quality case management within a managed care delivery system. Requires Master's degree in Nursing, Case Management, or Social Work and LCSW licensure; minimum ~5 years of relevant experience. Must demonstrate strong leadership, critical thinking, problem resolution, and time management, with knowledge of ACMA, CMSA, NASW Standards of Practice, InterQual criteria, MIDAS, and EHR systems. Responsibilities include directing services, project management, performance improvement (benchmarking/outcomes), collaboration with internal/external stakeholders, and adherence to CMS, Title 22, CHA Consent Manual, CDPH, and TJC standards; ability to lead teams and drive continuous quality improvement in a fast-paced environment.
Required Qualifications
