Job Summary
Provide day-to-day leadership, guidance, and support to the case management team while maintaining an active caseload to ensure high-quality, participant-centered services. Conduct assessments, develop service plans, and assist participants in accessing housing resources, benefits, healthcare, and employment services. Monitor case management documentation, review participant records for quality and completeness, and ensure timely, accurate HMIS data entry and maintenance. Provide coaching, case consultation, and mentorship to strengthen staff skills, while collaborating with program leadership and multidisciplinary teams to coordinate participant care. Complete functional timesheets, participate in reflective supervision sessions, and create documentation of job processes.
Required Qualifications
- Bachelor's degree in social work, human services, or related field preferred; equivalent experience may be considered
- Minimum of three years of experience providing case management services to individuals experiencing homelessness or housing instability
- Experience leading, mentoring, or supporting case management teams preferred
- Strong knowledge of HMIS and case management documentation requirements
- Experience working with diverse populations and individuals with complex needs
- Strong organizational, leadership, and communication skills
- In possession of a valid CA driver's license, proof of insurance, and be approved by RP insurance as a driver in order to drive agency vehicles
- Able to complete all required background checks
- Additional requirements may be deemed necessary, including fingerprinting (Live Scan), completion of all Community Care Licensing (CCL) documents, and ability to meet CCL licensing requirements
- Must be able to stand for long periods of time
- Must be able to perform physical activities, including lifting up to 25 pounds
- Local travel in the greater Bay Area may be required
- Applicants must be located in the greater Bay Area
Desired Qualifications
- Experience working in shelter, residential, or housing programs
- Knowledge of trauma-informed care, harm reduction, and housing-first approaches
- Experience supporting program compliance and quality assurance efforts
