Job Summary
Reconcile discharge medications, fill prescriptions, and coordinate bedside delivery for patients preparing to leave Ben Taub Hospital. Provide medication education before discharge and make follow-up calls to reinforce learning and answer questions. Oversee Pharmacy Technician Transition Specialists and support inpatient pharmacy operations when needed. Work closely with pharmacists, technicians, nurses, and physicians to ensure prescriptions are accurate, safe, and clearly understood. Make follow-up calls to reinforce education and answer medication-related questions.
Required Qualifications
- Bachelor's Degree in Pharmacy or PharmD from an accredited School of Pharmacy
- Current Texas Pharmacist License or eligibility to obtain licensure within 90 days of employment
- Basic Life Support certification from the American Heart Association or Red Cross within 90 days of employment
- IV Sterile Certification
- Minimum of 2 years of related pharmacy experience or completion of a PGY1 residency
- Strong verbal and written communication
- Exceptional verbal communication and public-speaking skills
- Ability to explain medication information clearly to patients and families
- Medication reconciliation and discharge planning
- Patient medication education
- Mathematics and medical terminology
- Advanced computer skills
- Experience with web-based tools, point-of-sale systems, and clinical systems
- Strong organizational and time-management abilities
- Creative problem-solving and excellent interpersonal skills

- Board of Pharmacy Specialties certification
