Job Summary
Clinical leadership and direct SUD patient care within an FQHC setting. Conduct comprehensive SUD assessments, develop and implement treatment plans, deliver evidence-based therapeutic interventions, and support Medication-Assisted Treatment (MAT/MOUD) programs. Coordinate care across behavioral health, primary care, and community-based services; promote recovery-oriented care and ensure compliance with regulatory and accreditation standards. Demonstrate strong communication, organizational, and leadership skills to improve patient outcomes, access to care, and population health.
Required Qualifications
- Physician (MD/DO), Nurse Practitioner (NP), or Physician Assistant (PA) degree from an accredited program
- Current unrestricted license to practice in the State of Minnesota
- Current DEA Certification
- Active BLS
- Minimum two years’ experience in providing substance use disorder (SUD) treatment plans
- 0-2 years’ experience in an FQHC or community health center or similar
